Situation

Health care's most pressing challenges require perspectives that extend beyond any single profession, department, or stakeholder group. To effectively address complex and interconnected issues, health systems must engage the collective wisdom and lived experiences of everyone affecting and affected by care, including patients, families, clinicians, staff, administrators, community partners, and policymakers.

Background

Patient and Family Advisory Councils (PFACs) have advanced meaningful patient engagement and strengthened shared decision-making across health care. However, traditional advisory models can unintentionally reinforce hierarchical structures in which some voices are perceived as more influential than others. Concerns about vulnerability, power dynamics, and organizational culture often limit transparent dialogue and authentic collaboration.

Assessment

Health care faces increasingly complex, or "wicked," problems that cannot be solved by isolated expertise alone. Lasting solutions require a culture in which transparency, trust, and shared responsibility are normalized. Just as the proverb reminds us that "it takes a village," meaningful change depends on engaging the entire community of stakeholders as partners in identifying challenges, designing solutions, and sustaining progress.

Recommendation

The Village Council model reimagines stakeholder engagement by recognizing every participant as a valued and equal contributor, not merely an advisor. Patients, families, clinicians, health care workers, executives, and community members come together as full stakeholders with a shared investment in the system's success.

By intentionally flattening traditional hierarchies, Village Council creates a space where diverse perspectives are respected, collective wisdom is leveraged, and responsibility for outcomes is shared. The model positions health care not only as a profession or institution, but as a vital community asset that belongs to and is shaped by those it serves. Through co-creation, mutual accountability, and inclusive leadership, Village Council fosters sustainable solutions that reflect the needs, aspirations, and expertise of the entire village.
